Eligible New Yorkers are:
  • Individuals Age 5 and older who
    • Reside in New York State, or
    • Work in New York State, or
    • Study in New York State
Proof of eligibility:
  • Residency: Current drivers license or other government issued photo ID
  • Employment status:
    • an employee ID card or badge
    • a letter from an employer or affiliated organization
    • a pay stub
  • Study: Student ID or other proof of enrollment.
  • Youth age 5-17: Parent or guardian/ caregiver must provide their ID and a signed consent form.

Appointment availability, vaccine brand, dose, and eligibility vary by pharmacy and are subject to change without notice. As such, it is important to check the pharmacy website before you go or make plans.

Individuals under age 18 must have a signed consent form and be accompanied by a parent or guardian.

Doctors, Pediatricians, Health Care Practitioners

  • Pediatric offices: Pfizer pediatric vaccine is available for any individual ages 5-11 years. Please call the individual practice for details.
  • Many family practices and health care practitioners have COVID-19 vaccine available. Please call your regular provider for information.

Flu Vaccine

Everyone age 6 months and older should get a flu shot to be immunized against influenza now. Visit your local pharmacy or your health care practitioner today. More info is here.

  • Distance--Please do not form lines or groups. Keep 6 feet of distance at all times
  • Density--Avoid entering crowded rooms or areas. Reduce occupancy to allow for proper distancing
  • Face Covering--You must wear a face covering when in public buildings and outdoor spaces around others
  • Hand Hygiene--Wash hands well and often. Clean and disinfect frequently touched surfaces often
  • Symptoms--If you have a fever, cough, shortness of breath, or body aches, stay home and get tested
