The Hazard Analysis Critical Control Point (HACCP) Inspection is an opportunity for the establishment to have the health department review food processes in the facility to make sure that all potential hazards are identified and to assure that the best food safety practices are being used.

Critical Violations

The violations listed below relate directly to factors that could lead to food-borne illness or injury. These violations generally involve the food source and condition, food cooking and storage temperatures, sanitary practices, water and sewage, and the use of poisonous materials.

Critical Violations were found at the following establishments:

Clarion Hotel, V-Lansing
Food was observed from unapproved source, spoiled, or adulterated on the facility premises.  Shredded cheese and tomato sauce were observed with mold.  The products were discarded during the inspection by Clarion staff.

Potentially hazardous foods were not stored under refrigeration.  Individual cream cheeses were observed on a counter at 50-60ºF for customer service. The products were discarded during the inspection in accordance with Technical Reference CEHFP 854.

A re-inspection was satisfactory.

Ithaca Ramada Inn Food Service, V-Lansing
Enough refrigerated storage was not properly maintained so that potentially hazardous foods were stored at 45ºF or below.  Various foods were observed in a cooler at 55-59ºF.  A repair-person was called during the inspection.  The food products were discarded by Ithaca Ramada Inn staff during the inspection in accordance with Technical Reference CEHFP 854.

A re-inspection was satisfactory.

Lansing Pizzeria, T- Lansing
Enough refrigerated storage was not properly maintained so that potentially hazardous foods were stored at 45ºF or below.  Cooked chicken wings and feta cheese were observed at 48-52ºF.  The products were discarded by Lansing Pizzeria staff during inspection and use of the refrigeration unit was discontinued.

A re-inspection was satisfactory.

Mahogany Grill, C-Ithaca
Accurate thermometers were not available or used to evaluate potentially hazardous food temperatures.  Thermometers present in facility were observed to be measuring 20-25 degrees too high.  The thermometers were calibrated during the inspection.

A re-inspection was satisfactory.

Stella’s, C-Ithaca
Potentially hazardous foods were not kept at or above 140ºF during hot holding.  Potatoes were observed at 110-115ºF.  The product was discarded during the inspection by Stella’s staff.

A re-inspection was satisfactory.

Southside Community Center Summer Feed Site (Re-inspection), C-Ithaca
Enough hot holding equipment was not properly maintained so that potentially hazardous foods were kept at or above 140ºF.  Hamburger patties were observed at 135ºF.  The product was rapidly re-heated to 165ºF prior to return to service.

A re-inspection was satisfactory.

Toad’s Diner, V-Freeville
Potentially hazardous foods were not stored under refrigeration.  Fresh shell eggs and cooked potatoes were observed on a counter at 70-80ºF.  The products were rapidly chilled to 45ºF or below prior to return to service.

A re-inspection was satisfactory.

Cinnamon Shoppe & Deli, T-Lansing
Enough refrigerated storage was not properly maintained so that potentially hazardous foods were stored at 45ºF or below.  Chicken, ham, turkey, and sliced tomatoes were observed at 52-53ºF.  The products were discarded in accordance with Technical Reference CEHFP 854.

A re-inspection was not conducted prior to submission of this report.

Simeon’s on the Commons, C-Ithaca
Cooked or prepared foods were subject to cross-contamination from raw foods.  Fresh shell eggs were observed stored over ready-to eat fruit, breads and other ready-to eat foods.  The storage was rearranged during the inspection.
A re-inspection was not conducted prior to submission of this report.

Taste of Thai, C-Ithaca
Foods or food storage areas were contaminated by sewage.  Waste water was observed on the floor in a room containing food storage freezers.  The operator stated that had occurred multiple times over the week prior to the inspection.  Repairs were being made during the inspection.  The facility was placarded and closed by the Health Department.

Potentially hazardous foods were not cooled by an approved method.  Cooked chicken was observed at 100ºF for customer service.  The product was discarded during the inspection in accordance with Technical Reference CEHFP 854.

Enough refrigerated storage was not properly maintained so that potentially hazardous foods were maintained at 45ºF or below during cold holding.  Eggs, bean sprouts, scallops, and squid were observed at 49-60ºF.  The products were discarded during the inspection in accordance with Technical Reference CEHFP 854.  A repair-person was called during the inspection.

A re-inspection was partially completed and no sewage was observed in the facility.  A re-inspection was satisfactory.

Boatyard Grill, C-Ithaca
Potentially hazardous foods were not kept at or below 45ºF during cold holding.  Chicken and steaks were observed at 51-53ºF in a cooler.  The products were placed in the walk-in cooler for rapid chilling to 45ºF or below prior to return to service.

A re-inspection was satisfactory.

Four Seasons, C-Ithaca
Potentially hazardous foods were not kept at or below 45ºF during cold holding.  Fresh shell eggs were observed at 53ºF.  The product was placed in the walk-in cooler for rapid chilling to 45ºF or below prior to return to service.

Potentially hazardous foods were not cooled by an approved method.  Cooked fish cakes were observed in a cooler for customer service at 105-110ºF.  The product was placed out for service without proper cooling.  The product was placed in a cooler for rapid chilling to 45º prior to return to service.

A re-inspection was not completed prior to submission of this report.

Souvlaki House, C-Ithaca
Potentially hazardous foods were not kept at or below 45ºF during cold holding.  Gyro meats and various cheeses were observed at 52-54ºF stacked in a cooler.  The products were rapidly chilled to 45ºF or below prior to return to service.

Potentially hazardous foods were not cooled by an approved method.  Ham was observed at 54-60ºF.  The product was placed in a sandwich unit for customer service prior to proper chilling.  The ham was placed in a cooler for rapid chilling to 45ºF or below prior to return to service.

A re-inspection was not completed prior to submission of this report.

Oakes Pizza I (Trumansburg Fair-Temporary Event), T-Ulysses
Potentially hazardous foods were at improper temperatures between 45 and 140ºF.  Cheese was observed at 55ºF in a cooler.  The product was rapidly chilled to 45ºF or below prior to return to service.

A re-inspection was satisfactory. 

Oakes Pizza II (Trumansburg Fair-Temporary Event), T-Ulysses
Potentially hazardous foods were at improper temperatures between 45 and 140ºF.  Cheese was observed at 55ºF in a cooler.  The product was rapidly chilled to 45ºF or below prior to return to service.

A re-inspection was satisfactory. 

Acapulco Mexican Grill (Trumansburg Fair-Temporary Event), T-Ulysses
Improper disposal of sewage and liquid waste was observed at the time of the inspection.  Grey water was observed running through a hose at the time of the inspection.

A re-inspection was satisfactory.

Coleman French Fries (Trumansburg Fair-Temporary Event), T-Ulysses
Potentially hazardous foods were at improper temperatures between 45 and 140ºF.  A hot holding unit containing chili and cheese was observed maintaining product temperatures at 92-103ºF.  The products were rapidly re-heated to 165ºF prior to return to service.

A re-inspection was satisfactory.

Oakes Soft Serve (Trumansburg Fair-Temporary Event), T-Ulysses
Potentially hazardous foods were at improper temperatures between 45 and 140ºF. Dairy based chocolate sauce was observed at ambient air temperature.  The product required refrigeration.  The vendor discarded the product during the inspection.

A re-inspection was satisfactory.
