The Hazard Analysis Critical Control Point (HACCP) Inspection is an opportunity for the establishment to have the health department review food processes in the facility to make sure that all potential hazards are identified and to assure that the best food safety practices are being used.

Critical Violations

The violations listed below relate directly to factors that could lead to food-borne illness or injury. These violations generally involve the food source and condition, food cooking and storage temperatures, sanitary practices, water and sewage, and the use of poisonous materials.

Critical Violations were found at the following establishments:

Second Landing, T-Lansing
Cooked or prepared foods were subject to cross-contamination from raw foods.  Fresh shell eggs were observed stored next to ready to eat cheese and sliced meats.  The storage was rearranged during the inspection.

Toxic chemicals were unlabeled in the facility.  The chemicals were labeled during the inspection.

A re-inspection was satisfactory.

Mehak Cuisine, C-Ithaca
Toxic chemicals were improperly stored.  Pesticide was observed stored in the foodservice area.  The pesticide was removed from the facility during the inspection.

Potentially hazardous foods were not kept at or below 45ºF during cold holding.  Cut melon was observed at 52-55ºF for customer service.  The product was placed in a cooler for rapid chilling to 45ºF prior to return to service.

Enough refrigerated storage was not properly maintained so that potentially hazardous foods were kept at or below 45ºF.  Cheeses, chicken and garbanzo beans were observed at 53-55ºF.  The products were discarded during the inspection according to Technical Reference CEHFP 854.

A re-inspection was satisfactory.

Little Thai House, C-Ithaca
Pre-cooked potentially hazardous foods were not reheated to 165ºF or above within two hours.  Rice was observed at 90-110ºF.  After two hours the product was observed at 120ºF.  The product was discarded according to Technical Reference CEHFP 854.

A re-inspection was satisfactory.

Rongovian Embassy to the USA, V-Trumansburg
Potentially hazardous foods were not reheated to 165ºF within two hours.  Black beans were observed at 99-120ºF after reheating for more than two hours.  The product was discarded according to Technical Reference CEHFP 854.

A re-inspection was satisfactory.

Ithaca Community Childcare Center, T-Ithaca
Cooked or prepared foods were subjected to cross-contamination from raw foods.  Fresh shell eggs were observed stored above melons for service.  The storage was rearranged during the inspection.

A re-inspection was satisfactory.

Garcia’s Mexican Restaurant, C-Ithaca
A cross-connection was observed during the inspection.  No back flow preventer was observed on a spray hose in the 3-bay sink.  The hose was raised above the sink top during the inspection.

Potentially hazardous foods were not stored under refrigeration.  Fresh shell eggs were observed stored on a counter at 64ºF.  The product was discarded in accordance with Technical Reference CEHFP 854 during the inspection.

A re-inspection was not conducted prior to submission of this report.

Blue Stone, C-Ithaca
Cooked or prepared foods were subjected to cross-contamination from raw foods.  Uncooked beef and fresh shell eggs were observed stored over lettuce and squash for customer service.  The storage was rearranged during the inspection.

Homewood Suites, V-Lansing
Potentially hazardous foods were not kept at or below 45ºF during cold holding.  Milk was observed at 49ºF.  The product was rapidly chilled to 45ºF or below prior to return to service.

A-1 Pizzeria, T-Dryden
Enough refrigerated storage was not properly maintained so that potentially hazardous foods were kept at or below 45ºF.  Heavy cream, alfredo sauce, and grated cheese were observed at 50-54ºF.  The products were discarded according to Technical Reference CEHFP 854 during the inspection.

Potentially hazardous foods were not kept at or above 140ºF during hot holding.  Chicken pieces were observed on a grill for customer service at 70ºF.  The product was rapidly re-heated to 165ºF prior to return to service.

Roy Josef Cuban Food Ithaca Farmers Market, C-Ithaca
Potentially hazardous foods were not cooled by an approved method.  Cuban sandwiches were observed in a cooler at 60ºF.  It was determined through interview that the sandwiches assembled at the commissary prior to going to Farmers Market and not cooled prior to placing them in a cooler for transport to the market for cooking. 

A re-inspection was not conducted prior to submission of this report.

Feel Good Foods, Throughout Tompkins County
Potentially hazardous foods were not kept at or above 140ºF during hot holding.  Chicken pieces were observed at 117-135ºF.  The product was voluntarily discarded during the inspection.

CU Flora Rose Dining, C-Ithaca
Potentially hazardous foods were not kept at or above 140ºF during hot holding.  Scalloped potatoes and vegetable soup were observed at 117-135ºF.  The potatoes were rapidly reheated to 165ºF prior to return to service.  The soup was voluntarily discarded during the inspection.

A re-inspection was not conducted prior to submission of this report.

Dryden Middle School, T-Dryden
Potentially hazardous foods were not kept at or above 140ºF during hot holding.  BBQ ham sandwiches were observed at 110-120ºF.  The product was rapidly reheated to 165ºF prior to return to service.
