The Hazard Analysis Critical Control Point (HACCP) Inspection is an opportunity for the establishment to have the health department review food processes in the facility to make sure that all potential hazards are identified and to assure that the best food safety practices are being used.

Critical Violations

The violations listed below relate directly to factors that could lead to food-borne illness or injury. These violations generally involve the food source and condition, food cooking and storage temperatures, sanitary practices, water and sewage, and the use of poisonous materials.

Critical Violations were found at the following establishments:

Buffalo Wild Wings, C-Ithaca
Toxic chemicals were improperly stored.  An unlabeled bottle of chemical was observed in a food service area.  The chemical was properly labeled and moved during the inspection.

A re-inspection was satisfactory.

CT Bento Café, C-Ithaca
Cooked or prepared foods were subject to cross-contamination from raw foods.  Fresh shell eggs were observed stored over pickled ginger and avocados for customer service.  The storage was rearranged during the inspection.

A re-inspection was satisfactory.

Culture Shock, C-Ithaca
Cooked or prepared food was subject to cross-contamination from raw foods.  Fresh shell eggs were observed stored over ready-to eat lettuce and tempeh.  The storage was rearranged during the inspection.

Accurate thermometers were not available to evaluate potentially hazardous food temperatures at the time of the inspection. The staff was directed to obtain an accurate metal-stem probe thermometer.

Toxic chemicals were improperly stored.  Sanitizer was observed on a table used for food preparation.
The product was moved during the inspection.

A re-inspection was not conducted prior to submission of this report.

CU Statler Banfi’s, C-Ithaca
Potentially hazardous foods were not stored under refrigeration.  Cut tomatoes were observed at 54ºF for customer service.  The product was rapidly chilled to 45ºF or below prior to return to service.

A re-inspection was satisfactory.

Dasan-J, C-Ithaca
Potentially hazardous foods were not stored under refrigeration.  Fresh shell eggs were observed on a counter at 60ºF.  The product was placed in a cooler for rapid chilling to 45ºF prior to return to service.

A re-inspection was satisfactory.

Freddy’s Place, T-Newfield
Accurate thermometers were not available to evaluate potentially hazardous foods.  No metal stem probe thermometer was in the facility at the time of the inspection.  The facility was advised to obtain an accurate metal stem probe thermometer.

A re-inspection was satisfactory.

Imperial Kitchen Buffet, V-Lansing
Adulterated food was observed on premises at the time of the inspection.  Pans of food were observed stacked inside each other so that the bottom of a pan was in food.  The pans were removed from the cooler, contaminated food was discarded; the remainder was covered and returned to the cooler.

Potentially hazardous foods were not stored under refrigeration.  Uncooked shrimp was observed on a counter at 53-55ºF.  The product was placed in a cooler to rapidly chill to 45ºF or below prior to return to service.  Pork wontons were observed on a tray at 66ºF.  The product was placed in a cooler for rapid chilling to 45ºF prior to return to service.

Potentially hazardous foods were not kept at or above 140ºF during hot holding.  Steamed clams, cooked broccoli, and Egg Foo Young were observed at 90-130ºF.  The products were discarded according to Technical Reference CSFP 854 during the inspection.  Products were replaced with freshly prepared foods and observed at 122-131ºF on the buffet.  These products were rapidly re-heated to 165ºF prior to return to service.

A re-inspection was satisfactory.

Insomnia Cookies, C-Ithaca
Cooked or prepared foods were subjected to cross-contamination from raw foods.  Fresh shell eggs were observed stored over sour cream and milk for customer service.  The storage was rearranged during the inspection.

Accurate thermometers were not available to evaluate potentially hazardous foods.  No probe thermometer was on site at the time of the inspection to evaluate potentially hazardous foods.

A re-inspection was satisfactory.

Maxie’s Supper Club & Oyster Bar, C-Ithaca
Potentially hazardous foods were not stored under refrigeration.  Beer batter, milk and egg mix were observed at 50-52ºF.  The batter and egg mix were rapidly chilled to 45ºF prior to return to service.  The milk was discarded during the inspection according to Technical Reference CSFP 854.

A re-inspection was not conducted prior to submission of this report.

Meadow Court Inn Food Service, C-Ithaca
Cooked or prepared foods were subject to cross-contamination from raw foods.  Fresh shell eggs were observed stored above tarter sauce for customer service.

Serendipity Catering, T-Ithaca
Potentially hazardous foods were not kept at or above 140ºF during hot holding.  Bean soup was observed at 125-130ºF.  The product was rapidly re-heated to 165ºF prior to return to service.

Stella’s Barn Restaurant, T-Newfield
Cooked or prepared foods were subject to cross-contamination from raw foods.  Raw beef patties were stored above ready-to serve coleslaw.  The storage was rearranged during the inspection.

A re-inspection was satisfactory.

Sunset Grill, T-Ithaca
Potentially hazardous foods were not stored under refrigeration.  Cooked potatoes were observed stored on a counter at 65-70ºF.  The products were discarded during the inspection according to Technical Reference CSFP 854.

A re-inspection was not conducted prior to submission of this report.

Tompkins County Jail, V-Lansing
Adulterated food was observed on the premises.  A block of cheese was observed with mold during the inspection.  The product was voluntarily discarded during the inspection.

A re-inspection was satisfactory.

DP Dough, C-Ithaca
Toxic chemicals were improperly stored.  A spray bottle with chemical was observed stored over a sink used for washing dishes.  The chemical was moved during the inspection.

Potentially hazardous foods were not stored under refrigeration.  Various cheeses were observed stored on a counter at 52-55ºF.  The products were discarded according to Technical Reference CSFP 854.

A re-inspection was not conducted prior to submission of this report. 

Jimmy John’s Gourmet Sandwiches, C-Ithaca
Toxic chemicals were improperly stored.  A bottle of chemical was not labeled.  The bottle was labeled during the inspection.

The Food Protection Program received and investigated eight complaints related to issues and/or problems at permitted food service establishments.  The complaints included food preparation and service, temperature abuse, accumulated garbage, and possible food related illness.
