Routine facility inspections are made without advance notice. It is important to keep in mind that inspections are only a “snapshot” in the entire year of a facility’s operation and they are not always reflective of the day-to-day operations and overall condition of the operation.

The Hazard Analysis Critical Control Point (HACCP) Inspection is an opportunity for the establishment to have the health department review food processes in the facility to make sure that all potential hazards are identified and to assure that the best food safety practices are being used.

Critical Violations

The violations listed below relate directly to factors that could lead to food-borne illness or injury. These violations generally involve the food source and condition, food cooking and storage temperatures, sanitary practices, water and sewage, and the use of poisonous materials.

Critical Violations were found at the following establishments:

Panera Bread, C-Ithaca
Potentially hazardous foods were not kept at or below 45ºF during cold holding.  Panini sandwiches were observed at 47-50ºF.  The products were placed in a cooler for rapid chilling to 45ºF or below prior to return to service.

A re-inspection was satisfactory.

Anna’s Vietnamese Restaurant, C-Ithaca
Potentially hazardous foods were not stored under refrigeration.  Uncooked Cornish hens were observed at 93ºF. in a pan of hot water.  The product was discarded during the inspection in accordance with technical reference CSFP 854.

A re-inspection was satisfactory.

Chili’s Grill & Bar, C-Ithaca
Potentially hazardous foods were not kept at or below 45ºF during cold holding.  Cooked chicken, cooked penne pasta, and meat batter were observed at 53-58ºF.  The chicken was rapidly chilled to 45ºF or below prior to return to service. The pasta and meat batter were discarded during the inspection in accordance with technical reference CSFP 854.

A re-inspection was satisfactory.

Taste of Thai Express, C-Ithaca
Potentially hazardous foods were not stored under refrigeration.  Rice noodles were observed at 63ºF.  The product was chilled to 45ºF or below prior to return to service.

A re-inspection was satisfactory.

CU-North Star, C-Ithaca
Adulterated foods were on the premises during the inspection.  One case of fresh ginger was observed with mold.  The product was voluntarily discarded during the inspection.

Enough refrigerated storage equipment was not properly maintained so that potentially hazardous foods were kept at or below 45ºF.  Pasta salad, potato salad, egg salad, mixed cut melon, and deli meat were observed at 49-50ºF.  The products were discarded during the inspection in accordance with technical reference CSFP 854.

A re-inspection was satisfactory.

Barnes & Noble Booksellers, C-Ithaca
Accurate thermometers were not available to evaluate potentially hazardous foods at the time of the inspection.  The probe thermometer in the facility at the time of the inspection read 40ºF on a counter.  A food worker calibrated the thermometer during the inspection.

Enough refrigerated storage equipment was not properly maintained so that potentially hazardous foods were at 45ºF or below.  Quiches, puddings, and cheese cake were observed at 53ºF.  The products were discarded during the inspection according to technical reference CSFP 854.

A re-inspection was satisfactory.

Subway #35087 Walmart, C-Ithaca
Potentially hazardous foods were not kept at or below 45ºF.  Pre-portioned meats were observed at 53ºF in a service cooler.  The products were discarded during the inspection in accordance with technical reference CSFP 854.

Potentially hazardous foods were not kept at or above 140ºF during hot holding.  Meatballs were observed at 123-142ºF for customer service.  Vegetable soup was observed at 135-136ºF.  The meatballs were discarded according to technical reference CSFP 854.  The soup was out of temperature less than two hours and was rapidly re-heated prior to return to service.

A re-inspection was satisfactory.

IC-Terrace Dining, T-Ithaca
Potentially hazardous foods were not kept at or above 140ºF during hot holding.  Cooked potatoes were observed at 120ºF.  The product was rapidly re-heated to 165ºF prior to return to service.

A re-inspection was not conducted prior to submission of this report.

Dryden Queen Diner, V-Dryden
Potentially hazardous foods were not kept at or below 45ºF during cold holding.  Sliced tomatoes were observed at 55-60ºF in a cold holding unit.  The product was rapidly chilled to 45ºF prior to return to service.

Potentially hazardous foods were not stored under refrigeration.  Fresh shell eggs were observed on a counter at 55-60ºF.  The product was rapidly chilled to 45ºF or below prior to return to service.

Wendy’s #1-005, C-Ithaca
Potentially hazardous foods were not kept at or below 45ºF during cold holding.  Shredded cheeses were observed at 50-51ºF.  The products were rapidly chilled to 45ºF or below prior to return to service.

ICSD-Northeast Elementary School, T-Ithaca
Cooked or prepared foods were subject to cross contamination from raw foods.  Fresh shell eggs were observed stored above deli meats.  The storage was rearranged during the inspection.

Miyake Japanese Restaurant, C-Ithaca
Food workers did not use proper gloves or utensils to eliminate bare hand contact with ready-to eat foods.  A food worker was observed handling lemons for customer service.  The product was voluntarily discarded during the inspection.

Other:

The Food Protection Program received and investigated four complaints related to issues and/or problems at permitted food service establishments.  The complaints included food preparation and service, temperature abuse, accumulated garbage, and possible food related illness. One complaint was received and referred to the Department of Agriculture and Markets.
