The Hazard Analysis Critical Control Point (HACCP) Inspection is an opportunity for the establishment to have the health department review food processes in the facility to make sure that all potential hazards are identified and to assure that the best food safety practices are being used.

Critical Violations

The violations listed below relate directly to factors that could lead to food-borne illness or injury. These violations generally involve the food source and condition, food cooking and storage temperatures, sanitary practices, water and sewage, and the use of poisonous materials.

Critical Violations were found at the following establishments:

Upstate Ladies AAU Girls Basketball Chicken BBQ, (Temporary Food Service), V-Lansing
Potentially hazardous foods were at improper temperatures.  Boiled potatoes were observed at 110ºF.  The product was rapidly re-heated to 165ºF prior to return to service.

A re-inspection was not conducted due to the one-day duration of the event.

Plum Tree Japanese Restaurant, C-Ithaca
Potentially hazardous foods were not stored under refrigeration.  Cooked shrimp were observed at 83-87ºF.  The product was rapidly chilled to 45ºF or below prior to return to service.

A re-inspection was satisfactory.

Ithaca Bakery, V-Lansing
Potentially hazardous foods were not kept at or below 45ºF during cold holding.  Cream cheeses were observed at 50-53ºF.  The products were voluntarily discarded during the inspection.

Potentially hazardous foods were not kept at or above 140ºF during hot holding.  Tomato bisque was observed at 123-127ºF.  The product was voluntarily discarded during the inspection.

A re-inspection was unsatisfactory.

Ithaca Bakery, (Re-inspection), V-Lansing
Potentially hazardous foods were not kept at or below 45ºF during hot holding.  Various cream cheeses were observed at 50-51ºF.  The products were voluntarily discarded during the inspection.

Board of Health action will follow.  

Mahogany Grill, C-Ithaca
Potentially hazardous foods were not kept at or below 45ºF.  Swiss cheese, gouda cheese, and cut tomatoes wee observed at 55-70ºF.  The products were voluntarily discarded during the inspection.

Enough refrigerated storage was not maintained so that potentially hazardous foods were kept at or below 45ºF.  Chicken and steaks were observed at 53-54ºF.  The products were rapidly chilled to 45ºF or below prior to return to service.

Pre-cooked, refrigerated foods were not re-heated to 165ºF or above within two hours.  Corn chowder was observed at 92-110ºF.  The product was voluntarily discarded during the inspection.

A re-inspection was satisfactory.

Dryden Baseball Chicken BBQ (Temporary Food Service Establishment), V-Dryden
Potentially hazardous foods were at improper temperatures.  Chicken was observed at 120ºF.  The product was rapidly re-heated to 165ºF prior to return to service.

A re-inspection was not conducted due to the one day duration of the event.

Seneca Place/Kilpatricks, C-Ithaca
Potentially hazardous foods were not kept at or below 45ºF during cold holding.  Half & half, cut cantaloupe, honey dew melon, and cut watermelon were observed at 50-51ºF.  The melons were rapidly chilled to 45ºF or below prior to return to service.  The half & half was voluntarily discarded during the inspection.

A re-inspection was not completed prior to submission of this report.

Kelly’s Dockside Kafe, C-Ithaca
Potentially hazardous foods were not stored under refrigeration.  Fresh shell eggs were observed at 82-84ºF on a counter.  The product was voluntarily discarded during the inspection.

Enough refrigerated storage equipment was not maintained to keep potentially hazardous foods at or below 45ºF.  Milk was observed at 52-53ºF in a cooler.  The product was voluntarily discarded during the inspection and a repair-person was called.

Potentially hazardous foods were not stored at 140ºF or above during hot holding.  Home fries and cooked potatoes were observed at 110-128ºF on a grill for customer service.  The products were voluntarily discarded during the inspection.

A re-inspection was not completed prior to submission of this report.

Collegetown Bagels Aurora Street, C-Ithaca
Potentially hazardous foods were not stored under refrigeration.  Swiss cheese was observed at 57ºF.  The product was rapidly chilled to 45ºF or below prior to return to service.

Wildfire, C-Ithaca
Enough refrigerated storage was not properly maintained so that potentially hazardous foods are kept at or below 45ºF.  Crab salad, crab cakes, salmon cakes, and cheeses were observed at 51-53ºF.  The products were voluntarily discarded during the inspection.

Country Inn & Suites Food Service, T-Ithaca
Accurate thermometers were not available at the time of the inspection to monitor potentially hazardous foods.  No probe thermometer was on site at the time of the inspection.

Potentially hazardous foods were not stored under refrigeration.  Cream cheese was observed at 70ºF on a counter.  The product was voluntarily discarded during the inspection.

A re-inspection was unsatisfactory.

Country Inn & Suites (Re-inspection), T-Ithaca
Potentially hazardous foods were not stored under refrigeration.  Cream cheese was observed at 54ºF on a counter.  The product was rapidly chilled to 45ºF or below prior to return to service.

Longview an Ithacare Community, T-Ithaca
Adulterated food was observed in the facility at the time of the inspection.  Moldy cheese was observed in a cooler.  The product was voluntarily discarded during the inspection.

Potentially hazardous foods were not kept at or below 45ºF.  Provolone cheese was observed at 56-58ºF.  The product was voluntarily discarded during the inspection.

Potentially hazardous foods were not cooled by an approved method. Fried cheese and cut tomatoes were observed at 50-63ºF.  The products were rapidly chilled to 45ºF or below.

Potentially hazardous foods were not cooled by an approved method.  Beef vegetable soup was observed at 60-62ºF.  The product was discarded during the inspection.

Toxic chemicals were improperly stored at the time of the inspection.  A chemical was observed stored above a food preparation area.  The storage was re-arranged during the inspection.

The Food Protection Program received and investigated three complaints related to issues and/or problems at permitted food service establishments. The complaints included food preparation and service, temperature abuse, accumulated garbage, and possible food related illness. One complaint was received and referred to the Department of Agriculture and Markets.
