Routine facility inspections are made without advance notice. It is important to keep in mind that inspections are only a “snapshot” in the entire year of a facility’s operation and they are not always reflective of the day-to-day operations and overall condition of the operation.

The Hazard Analysis Critical Control Point (HACCP) Inspection is an opportunity for the establishment to have the health department review food processes in the facility to make sure that all potential hazards are identified and to assure that the best food safety practices are being used.

Critical Violations

The violations listed below relate directly to factors that could lead to food-borne illness or injury. These violations generally involve the food source and condition, food cooking and storage temperatures, sanitary practices, water and sewage, and the use of poisonous materials.

Critical Violations were found at the following establishments:

Applebee’s Neighborhood Bar & Grill, V-Lansing
Potentially hazardous foods were not kept at or below 45ºF during cold holding.  Cheeses were observed at 55ºF.  The products were voluntarily discarded during the inspection.

A re-inspection was satisfactory.

Rogan’s Corners IC Concession, T-Ithaca
Potentially hazardous foods were observed at improper temperatures.  Breakfast pizza and breakfast sandwiches were observed at 112-125ºF.  The products were voluntarily discarded during the inspection.

A re-inspection was not conducted prior to submission of this report.

Joe’s Restaurant, C-Ithaca
Potentially hazardous foods were not kept at or below 45ºF during cold holding.  Feta cheese, tomatoes, and pesto were observed at 50-52ºF.  The products were voluntarily discarded during the inspection.

Potentially hazardous foods were not kept at or above 140ºF during hot holding.  Alfredo sauce was observed at 120ºF.  The product was rapidly re-heated to 165ºF.prior to return to service.

A re-inspection was satisfactory.

Miyaki Japanese Restaurant, C-Ithaca
Food workers prepared raw and cooked or ready-to-eat foods without thorough hand washing and sanitary glove use.  A food worker was observed cracking raw shell eggs and then handling cooked chicken for customer service.  The chicken was voluntarily discarded during the inspection.

Food workers did not use proper gloves or utensils to eliminate bare hand contact with ready-to-eat foods.  A food worker was observed handling lettuce for customer service.  The product was voluntarily discarded during the inspection.

Potentially hazardous foods were not kept at or above 140ºF during hot holding.  Fish stock was observed at 120ºF.  The product was rapidly re-heated to 165ºF prior to return to customer service.

Potentially hazardous foods were not stored under refrigeration.  Cooked shrimp tempura was observed at 90-100ºF.  The product was rapidly chilled to 45ºF or below prior to return to service.

A re-inspection was satisfactory.

Little Thai House, C-Ithaca
Potentially hazardous foods were not stored under refrigeration.  Tofu was observed at 98-120ºF on a counter.  The product was placed in a cooler for rapid chilling to 45ºF or below prior to return to service.

A re-inspection was satisfactory.

Vietnam/Hai Hong, C-Ithaca
Cooked or prepared foods were subject to cross-contamination form raw foods.  Heads of lettuce were observed stored next to raw meats.  The storage was re-arranged during the inspection.

Potentially hazardous foods were not kept at or below 45ºF during cold holding.  Sliced tomatoes were observed at 48-51ºF.  The product was voluntarily discarded during the inspection.

A re-inspection was satisfactory.

Country Club of Ithaca, T-Ithaca
Food workers did not use proper gloves or utensils to eliminate bare hand contact with ready-to-eat foods.  A staff person was observed handling breads for customer service.  The product was voluntarily discarded during the inspection.

A re-inspection was not conducted prior to submission of this report.

Dino’s Mediterranean Deli, C-Ithaca
Accurate thermometers were not available at the time of the inspection to evaluate potentially hazardous food temperatures.  The staff was directed to obtain an accurate metal stem probe thermometer.

Cooked or prepared foods were subject to cross-contamination from raw foods.  Fresh shell eggs were observed stored over dressings and bread for customer service.  The storage was rearranged during the inspection.

Potentially hazardous foods were not stored under refrigeration.  Spanikopita was observed on a counter at 64ºF.  The product was rapidly chilled to 45ºF prior to return service.

A re-inspection was not conducted prior to submission of this report.

Come To You Canteen, Throughout Tompkins County
Potentially hazardous foods were held at an unacceptable temperature.  Meatball and breakfast sandwiches were observed at 128ºF.  The products were voluntarily discarded during the inspection.

Potentially hazardous foods were observed at improper temperatures.  Milk and eggs were observed at 48-50ºF.  The products were rapidly chilled to 45ºF or below.

A re-inspection was not conducted due to the one day duration of the event.

Other:

The Food Protection Program received and investigated two complaints related to issues and/or problems at permitted food service establishments (FSE).  The complaints included food preparation and service, temperature abuse, accumulated garbage, and possible food related illnesses.

On November 8, 2009, Wendy’s #1-005, Elmira Road, City of Ithaca had a fire which was limited to the fryer area.  An extinguisher was discharged by staff and the fire department called.  After clean-up and servicing of the fryer unit the facility re-opened.
