Routine facility inspections are made without advance notice. It is important to keep in mind that inspections are only a “snapshot” in the entire year of a facility’s operation and they are not always reflective of the day-to-day operations and overall condition of the operation.

The Hazard Analysis Critical Control Point (HACCP) Inspection is an opportunity for the establishment to have the health department review food processes in the facility to make sure that all potential hazards are identified and to assure that the best food safety practices are being used.

The violations listed below relate directly to factors that could lead to food-borne illness or injury. These violations generally involve the food source and condition, food cooking and storage temperatures, sanitary practices, water and sewage, and the use of poisonous materials.

Critical Violations were found at the following establishments:

Queen of Tarts, C-Ithaca
Potentially hazardous foods were not kept at or below 45ºF during cold holding.  Quiche was observed at 55-58ºF.  The product was rapidly chilled to 45ºF prior to return to service.

Potentially hazardous foods were not stored under refrigeration.  Milk and ½ & ½ were observed on a counter for customer service.  The products were voluntarily discarded during the inspection.

A re-inspection was satisfactory.

East Shore Café, V-Lansing
Potentially hazardous foods were not cooled by an approved method.  Cooked noodles were observed at 53-56ºF after being in the cooler for more than six hours.  The product was voluntarily discarded during the inspection.

A re-inspection was satisfactory.

Lehigh Valley House, C-Ithaca
Cooked or prepared foods were subject to cross-contamination from raw foods.  Cooked chicken was observed stored in a box with raw chicken.  The storage was rearranged during the inspection.

Potentially hazardous foods were not cooled by an approved method.  Chili and cooked prime rib were observed at 49-57ºF after being in the cooler for more than twelve hours.  Both products were voluntarily discarded.

A re-inspection was satisfactory.

CU-Northstar, C-Ithaca
Enough refrigerated storage was not available or maintained so that potentially hazardous foods were maintained at 45ºF or below.  Sour cream, herb fresco, feta cheese, and cooked sausage were observed stored in a cooler at 50-55ºF.  The products were voluntarily discarded during the inspection.

A re-inspection was satisfactory.

Mandible Café, C-Ithaca
Potentially hazardous foods were not kept at or below 45ºF during cold holding.  Cream cheese and meat wraps were observed at 50-57ºF.  The products were placed in a cooler to rapidly chill to 45ºF or below prior to return to service.

Potentially hazardous foods were not stored under refrigeration.  Milk was observed at 56ºF on a shelf for customer service.  The product was voluntarily discarded during the inspection.

Potentially hazardous foods were not kept at or above 140ºF during hot holding.  Cooked rice was observed at 130-134ºF.  The product was voluntarily discarded during the inspection.

A re-inspection was satisfactory.

Heights Café, V-Cayuga Heights
Potentially hazardous foods were not kept at or above 140ºF during hot holding.  Cooked rice was observed at 115-125ºF.  The product was rapidly re-heated to 165ºF prior to return to service.

A re-inspection was not conducted prior to submission of this report.

Brotchen, C-Ithaca
Cooked or prepared foods were subject to cross-contamination form raw foods.  Fresh shell eggs were observed stored above rolls, tomatoes and herbs.  The storage was rearranged during the inspection.

Food Workers did not use proper gloves or utensils etc. to eliminate bare hand contact with ready-to eat foods.  A worker was observed handling tarts for customer service.  The product was voluntarily discarded during the inspection.

CU-Java City, C-Ithaca
Potentially hazardous foods were not kept at or below 45ºF during cold holding.  Panini meat and vegetable sandwiches, pre-made wraps, and salads with meat were observed at 51-54ºF.  The products were sold within one hour.

A re-inspection was not conducted prior to submission of this report.

CU-New York State Veterinary College, T-Ithaca
Potentially hazardous foods were not kept at or below 45ºF during cold holding.  American cheese, provolone cheese, and fresh shell eggs were observed at 50-64ºF.  The cheeses were rapidly chilled to 45ºF or below prior to return to service.  The fresh shell eggs were out of temperature more than two hours.  The product was voluntarily discarded during the inspection.

Potentially hazardous foods were not cooled by an approved method.  Cooked sliced chicken was observed at 104ºF for customer service.  The product was rapidly chilled to 45ºF or below prior to return to service.

A re-inspection was not conducted prior to submission of this report.

Dasan-J Restaurant, C-Ithaca
Enough refrigerated storage was bit maintained so that potentially hazardous foods are stored at 45ºF or below.  Chesses and tofu were observed at 52-55ºF.  The products were voluntarily discarded during the inspection.

A re-inspection was not conducted prior to submission of this report.

College Town Pizza, C-Ithaca
Accurate thermometers were not available to evaluate potentially hazardous foods.  No food thermometer was on site at the time of the inspection.  College Town Pizza staff was advised to obtain an accurate metal stem probe thermometer to evaluate food temperatures.

A re-inspection was not conducted prior to submission of this report.

Max’s Classic American Bar & Grill (Holiday Inn). C-Ithaca
Enough refrigerated storage equipment was not maintained so that potentially hazardous foods were kept at or below 45ºF.  Milk was observed at 50-52ºF.  The product was voluntarily discarded during the inspection.  

A re-inspection was not conducted prior to submission of this report.

Wendy’s #1-005, C-Ithaca
Enough refrigerated storage was not maintained so that all potentially hazardous foods were kept at or below 45ºF during cold holding.  Various cheeses were observed at 53-62ºF in a cooler.  The products were voluntarily discarded during the inspection.

A re-inspection was not conducted prior to submission of this report.

Lansing Fresh Café, T-Lansing
Accurate thermometers were not available at the time of the inspection to evaluate potentially hazardous foods.  No probe thermometer was in the facility at the time of the inspection.  Staff was directed to obtain an accurate thermometer.

Potentially hazardous foods were not stored under refrigeration.  Half & Half and milk was observed at 54ºF on a counter for customer service.

A re-inspection was not conducted prior to submission of this report.

TST BOCES Community School, C-Ithaca
Potentially hazardous foods were not stored under refrigeration.  Ham, turkey, chick peas, and cheese were observed at 53-64ºF.  The product was transported from the main school unrefrigerated. The products were rapidly chilled and served within one hour.

A re-inspection was not conducted prior to submission of this report.

Tompkins Cortland Community College, T-Dryden
Enough refrigerated storage was not maintained so that potentially hazardous foods were kept at or below 45ºF.  Cheese, hard boiled eggs, cream cheese, and macaroni salad were observed at 49-58ºF.  The products were voluntarily discarded during the inspection.

Hatfields Catering (Temporary Food Service Establishment), T-Lansing
Potentially hazardous foods were at improper temperatures.  Baked beans and baked potatoes were observed at 100-120ºF.  Both products were rapidly re-heated to 165ºF prior to return to service.

An accurate thermometer was not available at the time of the inspection.  No probe thermometer was on site at the time of the inspection.

A re-inspection was not conducted due to the short duration of the event.

West Danby Community Association Picnic (Temporary Food Service Establishment), T-Danby
Accurate food thermometers were not available at the time of the inspection to evaluate food temperatures.

Kiwanis Club of Ithaca Hawaiian Luau Fund Raiser (Temporary Food Service Establishment), C-Ithaca
Potentially hazardous food was at improper temperatures.  Macaroni salad was observed at 51ºF.  The product was rapidly chilled to 45ºF or below prior to return to service.  Cooked chicken was observed at 106-112ºF.  The product was rapidly re-heated to 165ºF prior to return to service.

A re-inspection was not conducted due to the short duration of the event.

Oakes Pizza Trumansburg Fair (Temporary Food Service Establishment), T-Ulysses
Accurate thermometers were not available to evaluate potentially hazardous foods.  The food thermometer on site at the time of the inspection measured eight degrees to high.  The thermometer was calibrated during the inspection.

A re-inspection was satisfactory.

Oakes Pizza II Trumansburg Fair (Temporary Food Service Establishment), T-Ulysses
Accurate thermometers were not available to evaluate potentially hazardous foods.  The food thermometer on site at the time of the inspection measured 6ºF to high.  The thermometer was calibrated during the inspection.

Potentially hazardous foods were at improper temperatures.  Nacho cheese sauce was observed at 90ºF.  The product was rapidly reheated to 165ºF prior to return to service.

A re-inspection was satisfactory.

Newfield Old Home Days Town of Newfield Recreation Committee (Temporary Food Service Establishment), T-Newfield
Potentially hazardous foods were at improper temperatures.  Baked beans were observed at 114-135ºF.  The product was voluntarily discarded during the inspection.

A re-inspection was not conducted due to the one day duration of the event.

Potentially hazardous foods were at improper temperatures.  Cooked hamburgers were observed at 115-130ºF.  The product was rapidly re-heated to 165ºF prior to return to service.

Fritz Vail Ithaca Apple Festival (Temporary Food Service), C-Ithaca
Potentially hazardous foods were at improper temperatures.  Cooked chicken spiedis were observed were observed at 120-125ºF. The product was rapidly re-heated to 165ºF prior to return to service.  Raw chicken spiedis were observed at 50ºF.  The product was rapidly chilled to 45ºF or below prior to return to service.

A re-inspection was satisfactory.

The Food Protection Program received and investigated five complaints related to issues and/or problems at permitted food service establishments.  The complaints included food preparation and service, temperature abuse, accumulated garbage, and possible food related illness.
