Our Roadmap to Health: Final Week to Take Community Health Survey

Tompkins County Whole Health (TCWH) invites the community to participate in our Community Health Survey, an opportunity to add your insights to the Tompkins County Community Health Improvement process (CHI Tompkins). The Community Health Survey is open to Tompkins County residents, ages 18 and over. Survey results are anonymous. The survey will run through the end of February 2025.
Director of Health Promotion Samantha Hillson stated, “The survey feedback we’ve received to date provides information about the interests and needs of the community. We have gotten a lot of valuable feedback from individuals 55 years and up from the Ithaca area. We are hoping to obtain additional input from the community, especially from those 18-34 years of age, before the survey closes at the end of the month. If you haven’t yet participated, this is your opportunity to add your feedback!”

- Older adults can complete the survey over the phone by calling County Office for the Aging (COFA) at 607-274-5482 during business hours 8:30 am – 4:30 pm.
Community Health Survey results are incorporated into the Community Health Assessment and Community Health Improvement Plan (CHIP), a local-level, community-informed outline for addressing priority health areas in Tompkins County. TCWH, along with Cayuga Health and other community partners, reviews the CHIP annually with community stakeholders, and conducts a county-wide Community Health Assessment Survey every 3-5 years.
The survey builds off of previous community engagement conducted earlier in the CHI Tompkins process, where community members shared their ideas for our Community Health Vision and Community Partner Assessment. The Community Health Survey is part of the second phase of the CHI Tompkins process, “Tell the Community Story.” A timeline for the process can be viewed on the website and resources will continue to be added, culminating with the publishing of the CHIP in 2026.
