Our Roadmap to Health: TCWH Launches Community Health Survey

Tompkins County Whole Health (TCWH) invites the community to participate in our Community Health Survey, an opportunity to add your insights to the Tompkins County Community Health Improvement process (CHI Tompkins). The survey will run through February 27, 2025.

Community Health Survey results are incorporated into the Community Health Assessment and Community Health Improvement Plan (CHIP), a local-level, community-informed outline for addressing priority health areas in Tompkins County. TCWH, along with Cayuga Health and other community partners, reviews the CHIP annually with community stakeholders, and conducts a county-wide Community Health Assessment Survey every 3-5 years.

The survey builds off of previous community engagement conducted earlier in the CHI Tompkins process, where community members shared their ideas for our Community Health Vision and Partner Assessment. The Community Health Survey is part of the second phase of the CHI Tompkins process, “Tell the Community Story.” A timeline for the process can be viewed on the website and resources will continue to be added, culminating with the publishing of the CHIP in 2026.

TCWH Commissioner Frank Kruppa stated, “The Community Health Survey provides valuable information about the community’s perspectives on health in Tompkins County. While completing the CHIP is a requirement for all local health departments, in Tompkins County we are invested in making this process meaningful and instructive to the local health system. We invite everyone to participate in our Community Health Survey; your engagement improves our work.”

Director of Health Promotion Samantha Hillson stated, “The Community Health Survey is an exciting milestone in the Community Health Planning process. Hearing from county residents about what they see as priorities allows us to focus our work on what matters most to the community. Your input makes a difference – thank you for your support in creating a healthier future together.”

The Community Health Survey is open to Tompkins County residents, ages 18 and over. Survey results are anonymous. The survey will run through February 27, 2025.

  • Paper version and translation (Chinese Traditional and Simple, French, Spanish, Russian, Karen) available by request through email: CHItompkins@tompkins-co.org or phone: 607-274-6600.
  • By phone: If you do not have internet access or need help completing the survey, dial 2-1-1 (1-877-211-8667) for assistance.
  • Older adults can complete the survey over the phone by calling County Office for the Aging (COFA) at 607-274-5482 during business hours 8:30 am – 4:30 pm.
