Community Health Assessment and Health Improvement Plan Released

(ITHACA, N.Y., December 4, 2013) — The Tompkins County Health Department released the 2013–2017 Community Health Assessment (CHA) and the Community Health Improvement Plan (CHIP). The CHA is a profile of the current health of Tompkins County residents based on socioeconomic factors, an assessment of key health indicators, local data, community resources and gaps, and information provided by a large and varied group of community partners.

The CHIP identifies two priorities for community focus based on community input and data analysis: Promoting Mental Health and Preventing Substance Abuse is one priority, and Preventing Chronic Disease, the second. The CHIP is built around continuing efforts in these areas and suggests new ideas to help the community to improve its overall health.

The CHA and CHIP can be found on the Health Department’s website: http://www.tompkinscountyny.gov/health/pnc/cha
